Hypothyroidism — Symptoms, Causes and Diagnosis
What does it mean to have an underactive thyroid, and how can it affect your health?
In this episode of the Endocrine Echo Lounge, I explain hypothyroidism in simple, easy-to-understand terms, exploring its common symptoms, causes, and diagnosis.
We discuss how hypothyroidism can cause fatigue, feeling cold, constipation, dry skin, difficulty concentrating, and weight changes—and why these symptoms do not always mean that your thyroid is the problem.
I also explain Hashimoto's thyroiditis, a common cause of hypothyroidism, and how other forms of thyroiditis can sometimes cause temporary changes in thyroid function. You'll learn why thyroid blood tests are essential for diagnosis and why symptoms alone are not enough to determine whether your thyroid is underactive.
Whether you're experiencing symptoms, have recently learned that your thyroid tests are abnormal, or simply want to understand thyroid health better, this episode offers a practical, evidence-based introduction to hypothyroidism.

Episode 3: Making Sense of Thyroid Tests — Understanding the Patterns
What can your thyroid blood tests actually tell you?
In this episode of the Endocrine Echo Lounge, Doctor Henry explains how to look at TSH, Free T4, and T3 together to understand what may be happening with the thyroid.
We’ll explore the common patterns seen with hypothyroidism, hyperthyroidism, subclinical thyroid disease, normal thyroid function, and less common patterns that may require further evaluation.
For example, a high TSH does not always mean the same thing. If the Free T4 is normal, the pattern may suggest subclinical hypothyroidism. If the Free T4 is low, it points toward a different thyroid problem. The important lesson is that thyroid tests need to be interpreted together rather than by looking at one number alone.
We’ll also discuss why TSH is usually the starting point, when Free T4 and T3 are helpful, and what doctors consider when thyroid results don't seem to fit together.
The goal is to make thyroid testing easier to understand and help you better understand what your own thyroid results may be telling you.

in this episode of the Endocrine Echo Lounge, Doctor Henry explains how thyroid testing is used to evaluate thyroid function and uncover the cause of thyroid disease.
We take a practical look at TSH, free T4, and T3, including why T3 can be especially helpful when evaluating hyperthyroidism and T3 thyrotoxicosis. We also discuss TPO antibodies, thyroglobulin antibodies, and TRAb, including the important distinction between stimulating, blocking, and neutral TSH-receptor antibodies and when a specific test such as TSI may be appropriate.
The episode also explains why reverse T3 is generally not useful in routine thyroid evaluation and discusses factors that can interfere with thyroid blood tests, including biotin, medications, changes in thyroid-binding proteins, and laboratory assay interference.
Most importantly, we discuss why thyroid tests should not be interpreted as isolated numbers. The results need to be considered together with the patient's symptoms, medications, supplements, examination, and overall clinical picture.
If you've ever wondered what your thyroid blood tests actually mean, this episode provides a straightforward, endocrinology-based guide to understanding the basics of thyroid testing.

In this episode of the Endocrine Echo Lounge, Doctor Henry explains how osteoporosis treatment decisions are made from an endocrine perspective. Rather than focusing only on the T-score, we look at overall fracture risk, FRAX, previous fractures, other medical conditions, and medications that can contribute to bone loss.
Doctor Henry discusses the major osteoporosis treatments, including bisphosphonates, denosumab (Prolia), teriparatide (Forteo), abaloparatide (Tymlos), and romosozumab (Evenity). The episode explains when these medications may be considered, important limitations and contraindications, and why some patients may need a bone-building medication followed by an antiresorptive treatment.
You'll also learn about bisphosphonate drug holidays, why denosumab should not simply be stopped, considerations surrounding dental procedures while taking denosumab, and how medications such as glucocorticoids and aromatase inhibitors can affect bone health.
Most importantly, this episode explains why osteoporosis treatment isn't one-size-fits-all—and why the goal is not simply to improve a DEXA number, but to prevent fractures and preserve strength, mobility, and independence.

in this episode of Endocrine Echo Lounge, Doctor Henry explains what happens to your metabolism during and after menopause—and why the changes you may notice are about more than simply getting older.
Why does weight seem to shift toward the abdomen? Why can maintaining muscle become more difficult? What happens to blood sugar, insulin sensitivity and cholesterol? And why does cardiovascular risk change during and after menopause?
Doctor Henry takes a closer look at the connection between menopause, body composition, visceral fat, muscle health, blood sugar, cholesterol and cardiovascular risk. He also discusses the importance of resistance training, physical activity, nutrition and sleep as part of maintaining metabolic health during midlife and beyond.
Most importantly, this episode focuses on what you can do to protect your muscle, support healthy metabolism, and reduce your long-term risk of diabetes and cardiovascular disease.
In this episode, you'll learn about:Menopause and abdominal fat; changes in body composition; muscle loss and why muscle preservation matters; insulin sensitivity and blood sugar; changes in cholesterol; cardiovascular risk during midlife; the role of resistance training and physical activity; nutrition and adequate protein; the importance of sleep; and why menopause can be an important opportunity for prevention.

In this episode of Endocrine Echo Lounge, Doctor Henry explains the evidence behind treating menopausal symptoms and how hormone therapy fits into the management of menopause.
The episode explores menopausal hormone therapy (MHT), including the role of estrogen, when treatment may be appropriate, and the important differences between oral and transdermal estrogen. It also discusses when progesterone or a progestogen is needed and why treatment should be individualized based on a woman's symptoms, medical history, and risk factors.
Doctor Henry also addresses non-hormonal treatment options, herbal therapies, bioidentical hormones, and testosterone therapy, helping separate evidence-based treatments from claims that may not be supported by strong scientific evidence.
The episode also puts the findings of the Women's Health Initiative (WHI) into perspective and explains how our understanding of hormone therapy has changed over time.
The goal is not to tell every woman that she should or should not take hormones, but to help women understand the benefits, limitations, and risks of the available treatment options so they can have a more informed conversation with their healthcare provider.
In the next episode, Doctor Henry will move beyond symptom treatment to discuss the broader effects of menopause—including metabolic changes, cardiovascular health, and sexual health.

Perimenopause: What Every Woman—and Her Partner—Should Know
In this episode of the Endocrine Echo Lounge, Doctor Henry explains what perimenopause is, when it begins, and why the transition can look different from woman to woman.
We discuss the common symptoms of perimenopause, how the diagnosis is made, and the important hormonal changes that occur as the ovaries gradually become less predictable. We also explore how these changes can affect sleep, mood, menstrual cycles, weight, metabolism, and overall well-being.
Most importantly, this episode helps separate what is a normal part of the menopausal transition from symptoms that may require further evaluation.
This conversation is not just for women. Perimenopause can affect relationships, families, and partners, making understanding and communication an important part of the journey.
Takeaway: Perimenopause is a natural transition, but that does not mean women should simply suffer through their symptoms. Understanding what is happening—and knowing when to seek medical guidance—can make this stage of life much easier to navigate.
